Deadlines

• Pre-registration closes February 27, 2009.
• On site registration opens March 6, 2009.

Payment

• Payment must be made at the time of registration.
• Payment must be in U.S. Currency, and checks must be drawn on a U.S. bank.

Cancellation & Refund Policy

• Cancellations must be made in writing and sent by e-mail to newagent@uthscsa.edu or faxed to 210-567-6072.
• Cancellations received on or before February 20, 2009, will be refunded less a 20% processing fee.
• Cancellations received after February 20, 2009, are non-refundable.

Name Change Policy

• Name changes must be made in writing and sent by e-mail to newagent@uthscsa.edu or faxed to 210-567-6072.
• Name changes received after February 27, 2009, will be processed on site.

Confirmations

• Online

              Acknowledgment of receipt of your registration will be e-mailed to you within a

              few minutes of completion of your online form.

              After payment has been processed, you will receive e-mail confirmation of your

              completed registration. This e-mail serves as your REGISTRATION RECEIPT.

• Fax & Mail

              After payment has been processed, you will receive e-mail confirmation of your

              completed registration. This e-mail serves as your REGISTRATION RECEIPT.

Press Registration

• There is no charge for press registration.
• Valid credentials are required.
• To register, please mail or fax your completed registration form and a copy of your credentials to:
